New Patient Registration Forms

As part of the transition to new healthcare providers from April 1st 2025, patients are required to complete new registration forms. Other Options to assist you in finding a family doctor

If you do not wish to continue your care at these clinics, you are welcome to search for a provider of your choice. Here are some suggestions to assist you with your search:

1. Ask family and friends if they can try and get you in with their doctor.

2. Speak to your local pharmacist who may be aware of new physicians in your area.

3. Call “Health Care Connect” at 1-800-445-1822 or online at https://www.ontario.ca/page/find-family-doctor-or-nurse-practitioner
