Have you completed and returned your enrollment form to our clinic?
All our patients are now required by the Ontario Ministry of Health to complete and return enrollment forms to our clinic timely. As we restructure our practice and welcome new patients by mid-October we strongly encourage all our patients who have previously registered with us to ensure they pick up our enrollment forms and return the completed forms to the clinic to avoid being DE-Registered. Please be aware that these forms are not the new patient application forms completed when registering with us. Thank you.
